Become A Client

 Interested in becoming a CWC client?

Follow the steps below:

1. Complete the "Counseling Screening Form"

 From a secure email (check your email security settings), 

Send a request to: Journey@counselingwithcarolyn.com 

Please enter "Counseling Request" in the subject line of your email. In the body, kindly provide your name, Florida county of residence, a reliable contact number, and your preferred time for phone calls (morning, noon, or evening). After I receive your information, I'll email you time slots for a complimentary 15-minute phone consultation.

2. Availability and Matching

If your current needs and availability are a match for CWC, Intake packet including Informed Consent, and next steps will be provided via email and sessions can begin promptly. CWC will offer feedback if your needs and schedule are a better fit with another provider.
